Karen, une jeune psychiatre, réussit à attraper le dernier métro juste avant l'interruption du trafic. Soudain, la rame s'arrête au milieu d'un tunnel, et le cauchemar commence... Plusieurs passagers reçoivent un mystérieux signal et se mettent à assassiner les autres voyageurs. Qui sont-ils ? Que veulent-ils ? A quelle mystérieuse organisation appartiennent-ils ?Karen et un petit groupe de survivants s'enfuient à travers les souterrains du métro, poursuivis par les tueurs...
